The skin around the eyes and all over the body is composed of three distinct layers.  The epidermis is the uppermost layer.  It is the skin that you see when you look in the mirror.  It is composed mainly of cells called keratinocytes, and acts as a shield against the external environment, and toxins.  The dermis is the next layer, and is up to 10 times thicker than the epidermis.  It is composed of a multitude of different elements including collagen, elastic fibers and ground substances (i.e. Hyaluronic acid).  The last layer of the skin is made up of subcutaneous fatty tissue.

There are many changes that occur to the skin as we all age.  The aging process is usually broken down into intrinsic and extrinsic changes.  The intrinsic portion of the aging process is secondary to genetic changes, and results from the passage of time.  Extrinsic aging is caused by sun exposure, pollution, stress, tobacco use, poor nutrition and hormonal changes throughout life.  Some estimates suggest that up to 80% of facial aging changes are secondary to sun exposure.

These changes lead to a characteristic picture of photoaged skin.  The manifestations of which include fine lines, wrinkling, mottled pigmentation, and loss of elasticity and translucency.  Patients with lighter skin tones are particularly susceptible to extrinsic photoaging.
